This form is designed to help the Information Technology Center (ITC) anticipate projects, potential IT as part of a capital request, or increases in FTEs or ITUs (Information Technology Users) for other departments in the upcoming fiscal year(s) that will involve ITC resources.

If you have projects, potential IT as part of a capital request, or increases in FTEs or ITUs that involves IT hardware and/or software please use the form below to submit the project to ITC. It is recommended that if there is any possibility that information technology (hardware, software, and/or service) is part of a project or upgrade that you submit this form. This would also include peripheral devices such as plotters, printers, scanners, cameras as well as technology services, such as hardware monitoring or software installation, provided by third parties. We can discuss the specifics of the project, potential IT as part of a capital request, or increases in FTEs/ITUs after this form is submitted. Please use this form for a single project, if you have more than a single project please submit subsequent forms.

If a Department contacts ITC for projects, potential IT as part of a capital request, or increases in FTEs/ITUs support that were not submitted using this form, the item(s) in question may be delayed based on the timing of other projects. Projects that have a public safety factor, whether submitted using the form or not, will be given precedence over other projects.

This form is not to be used as a request for replacement or upgrades to desktops, laptops, or printers that you or your department are currently using. Those items are replaced or upgraded on a case by case basis throughout any fiscal year.
