New Employees

Prior to beginning employment with the Town, you will be sent an offer letter from the Human Resources Department and instructed to complete all necessary pre-employment forms. All new hires will need to complete the first group of forms below (1-9), as well as any additional forms applicable to the employment type shown in the subsequent groups below. Most employment offers are conditioned upon complete, satisfactory and timely submission of these forms prior to employment.

All New Hires
1W-4 Income Tax Withholding (suffices for both State and Federal withholdings)
2I-9 Employment Eligibility Verification (must include supporting documentation – see list of acceptable documents)
3Direct Deposit Form (must include voided check or banking form)  
4Conflict of Interest Law for Municipal Employees (must register online and take test - follow link within)
